NAEH Coordinated Assessment Toolkit

 

The National Alliance to End Homelessness (NAEH) has developed a toolkit to help organizations to understand, plan, implement and evaluate coordinated assessment systems. Coordinated assessment allows organizations to more efficiently assist the individuals and families who are homeless or are on the brink of being homeless by more effectively and efficiently matching people to the services they need and improving data collection.

The coordinated assessment toolkit contains 4 sections:

  • Planning and Assessment
  • Data and Implementation
  • Evaluation
  • Community Examples and Materials
