CHESS Funding Approved
CT received CMS approval to launch the Connecticut Housing Engagement and Support Services (CHESS) program which will bring coordinated healthcare and housing support to individuals with mental health, substance use, and other serious health conditions.
CHESS is aimed at managing the difficulties that are often part of homelessness, including access to healthcare and handling chronic health issues, with the overall goal of promoting health and well-being by coordinating targeted healthcare with housing. Additional information about CHESS can be found here on the DSS website. DSS has opened applications through www.CTCHESSDSS.com. This site also includes a helpful brief summary of the program, who is eligible, how it works, what support is available, timelines, etc. For specific questions, providers or participants can contact DSS directly: (888) 992-8637.
